Membership means
This is an in-office savings plan, not dental insurance, and applies to services provided at Fort Dental only. Cosmetic procedures — including veneers, clear aligners, and cosmetic bonding — are not included in the plan or the member discount. Ask our team for full plan terms.

No. It’s an in-office savings plan — a direct agreement between you and Fort Dental. There are no claim forms, deductibles, waiting periods, or yearly maximums, and it applies to services provided at Fort Dental only.
Your annual membership covers your preventive care: a comprehensive evaluation, a full set of digital X-rays, two routine cleanings, additional exams as needed, two fluoride treatments, an oral cancer screening, a full-mouth digital scan, and 3D Cone Beam CT imaging when needed.
Those aren’t part of the preventive membership, but members receive 20% off our standard fees for restorative care (fillings, crowns, bridges, and implant restorations), periodontal treatment, nightguards and retainers, additional cleanings, and take-home products.
No — cosmetic procedures such as veneers, clear aligners, and cosmetic bonding are not included in the membership and aren’t eligible for the member discount.
Yes. Each family member enrolls individually at $500 per year. You can sign up in office or over the phone, and your membership is active right away. If you cancel within 30 days and haven’t used any services, we’ll refund your fee in full.
